Introduction Starting from the ancient times human was always interested in strange phenomenon of sleeping and especially in dreams. Dreams were described in different ways. For instance, dreams were explained psychologically like images of sub consciousness and feedback of neural processes in human’s brain. Spiritually, it was described like messages of god (C.S. Lewis, nd). Remarkable that not only human have ability to see dreams but several species of animals (Wilkerson R. 2003). For example, rat’s sleep is almost same as the human’s they have multiple stages of sleep starting from slow wave sleep to REM stage of sleep (Animals have complex dreams, 2001). Another fact is that living beings, including humans, have more REM sleep in younger age (Siegel, 2005). In summary, dreams are well known for strange, amazing and impossible thing to happen in them, but almost all people don’t actually realize that they are sleeping during the dream. To conclude, dream itself co uld be defined as the complexity of images and emotions that occur during certain stages of sleep ("Dream". The American Heritage Dictionary of the English Language, Fourth Edition. 2000). It is proved that dreams are linked with rapid-eye movement (REM) stage of sleep which occur every 1.5-2 hours of sleeping and with time progress they become longer. The REM stage usually includes rapid eye movement, heavier breath, increased pulse rate and temporary weakening of muscles (Dream Basics: Understanding Sleep, 2007). It seems that dream can happen not only in REM stages of sleep, but it’s easier to remember a dream by awaking during the REM stage (Dement 1957; Kleitman 1957; Hobson 2009). It is noticed that if the person lives with emotionally rich life and his hormon... .... Preface Since the early customs union, the European Union has been keen to create a single market for its member states. In this assignment, I am going to discuss to what extent the European Union has created a single integrated economy for its member states. 2. Introduction From the early Customs Union to the Single European Market to the recent Single European Currency—the Euro, the European Union achieved a lot of successes in the process of creating a single integrated economy for its member states in the last fifty years. Nonetheless, with so many successes, the European Union still have a long way to go to create a true single economy for its member states. For example, there are still significant differences of corporate governance regimes and social/labour models between member states. And the enlargement towards Central and Eastern Europe is another incomplete big task for the European Union. Major Achievements 3.1 Customs Union and The Early Enlargement In 1948, the Benelux--a customs union in industrial goods was formed by the Netherlands, Belgium and Luxembourg. After the Treaty of Paris and the Treaty of Rome were signed, the customs union comprised Benelux, France, Germany and Italy and was extended to all industrial goods (not just for coal and steel). In 1969, the customs union completed the elimination of customs duties and quantitative restrictions on the import and export goods between the original six. (McDonald D, 1999) This was the most basic economic arrangement of the EU. Between the elimination of tariffs and quotas and the agreement of establishing the Single European Market, there was very little progress towards the single market. (McDonald D, 1999) But enlargements were very successful with member states growing from 6 to 15 which included Denmark, Ireland and the United Kingdom joined in 1973, Greece in 1981, Spain and Portugal in 1986 and Austria, Finland and Sweden in 1995. (Europa, 2003a) 3.2 The Single European Market In 1985, the European Commission chaired by Jacques Delors published a White Paper proposing the member states for completing the internal market. This white paper made it clear that there were three types of barriers (physical, technical and fiscal) standing in the way of the completion of the internal market. (Mercado S. et al, 2001) In the next year, 12 member states signed the Single European Act which contained a blueprint and a timetable for adopting over 270 liberalising measures which would be necessary for the creation of a single market. Free Will and Divine Foreknowledge Essay

Free will is the ability to make our own choices in issues regarding all aspects of life. It is a power that enables us to make our own choices that are not affected by external factors such as divine will. Therefore, each one sins by his/her own will. While, divine foreknowledge is the fact that God has complete knowledge of what will happen in the future. In â€Å"On Free Choice of the Will†, St. Augustine discusses a critical issue which is the incompatibility of man’s free will and God’s foreknowledge. So the question is, do we really have free will in spite of the fact that God foreknows everything? If God knows what must necessarily happen next, then how do have the free will to make our own choices? Augustine comes up with a series of arguments to prove that we sin by our own will with no intervention of the divine foreknowledge. Augustine first argued a characteristic of God that He has free will, and that He has foreknowledge of his own actions. Therefore, both God’s will and foreknowledge go along with each other. From this point he then assumes that man’s will and God’s foreknowledge are both compatible. But can we compare God with man? And is this argument convincing enough? More elaboration has to be given in order to make it more convincing. Augustine then proceeds to do so. He states that people who do not believe in the compatibility of free will and divine foreknowledge are those who â€Å"are more eager to excuse than confess their sins† (p. 73). That means that people who always blame others for their own wrong doings rather than admitting it are those who claim that we have no free will and that everything is already known by God, and that nothing can be changed, which they also use as a justification for their wrong actions. These people live their life by chance, leaving everything according to the circumstances rather than trying to take good actions. An example for that is the beggars, who always try to take money from people without giving anything in return or even having a job, although they have the ability to do so. But because of their laziness and their belief that this is what they were created to be, they leave everything to happen by luck and according to God’s foreknowledge that couldn’t be changed (p. 73). Augustine then moves to another point which is the relation between the will and the power to achieve that will. He states that the will itself is within our power. Therefore, our desire to commit certain acts is a power that we own. But if we will something that is not within our power then it is not considered as a will because we can only will what is within our power. Augustine then discusses that if something good happens to us then it is accordance to our will, not against it. So for example, being happy, although God foreknows that you will be so, doesn’t mean that we are happy against our will. Thus, God’s foreknowledge of our happiness doesn’t take away our will to be happy (p. 76). And so, he concludes that if God foreknows our will, then definitely this will is going to occur, and so it will be a will in the future. Consequently, his foreknowledge doesn’t take away our will. And since that what we will is in our power, God foreknows our power and He will not take it away. Hence, we will have that power because God foreknows it (p. 77). So Augustine made it clear â€Å"that it is necessary that whatever God has foreknown will happen, and that he foreknows our sins in such a way that our wills remain free and are with in our power† (p.77). However, the fact that God’s foreknowledge of our sins is consistent with our free will in sinning still stays questionable. Taking into consideration the fact that God is just, so how does He punish our sins that happen by necessity? Or is God’s foreknowledge not an obligation? The topic is still confusing so Augustine then proceeds to make it clearer. He explains that if we are certain that someone is going to sin, then we have foreknowledge with the wrongdoing that he/she is going to commit. This foreknowledge didn’t force them to do so, but it was done by their own free will. Accordingly, their will to sin is consistent with our foreknowledge of that sin. Therefore, â€Å"God forces no one to sin, even though he foresees those who are going to sin by their own will† (p. 78). Augustine then compares foreknowledge with memory. He states that our â€Å"memory does not force the past to have happened†, and similarly God’s foreknowledge of the future doesn’t force it to occur (p. 78). And we remember things in the past that we have done but didn’t do everything that we remember, likewise God foreknows everything that He will cause in the future, but doesn’t cause everything that is within His foreknowledge (p. 78). As a result, God punishes our sins that we do by our own will and which He did not cause, as God is known by his justice. Augustine then comes up with a good argument for all those who are still slightly confused, that if God should not punish us for our sins that He foresees then He also shouldn’t reward us for our good work that He also foresees (p. 78). To conclude, Augustine succeeded in coming up with a good argument showing that man’s free will and God’s foreknowledge are both compatible. The sequence of his ideas made his argument understandable and convincing for any reader. As a reader, I’ve always thought about that subject but didn’t receive any answers. However, reading â€Å"On Free Choice of the Will† made everything clear for me and made me well convinced that God’s foreknowledge doesn’t intervene with our own choices that we make. Since Germany, leadership was weak and Hitler had already dismissed his commander in chief, this counter attack swept them off the ground and the result was that hundreds of thousands of their military men were killed though the dam ages and killings were both way traffic. Conclusion The Soviet Union victory against Germany was the beginning of liberation of humankind from the hands of a dictator Hitler. The Germans were thought to be the most powerful nation in the world and even at the verge of their defeat; they still under estimated the Russian resources. The Russians did their preparation for the counter attack very carefully, putting every detail of their internal organization in perspective, as they knew that a slight mistake would bestow victory upon their rivals. On the other hand, by the time, Germans realized the ability in Russia, there was little left to do apart from surrendering. Furthermore, the nature of dictatorship in Hitler cost Germany a great deal since he dismissed the opinions of professionals making it hard for them to operate. After giving you a list of every college that superscores the SAT, I'll talk about how you can use this information strategically as you prepare to apply to college. To start, let's briefly review what "superscoring" means. What Is an SAT Superscore? Superscoring is the process by which colleges consider your highest section scores across all the dates you took the SAT. Rather than confining your scores to one particular date, these schools will take your highest section scores, forming the highest possible composite score. On the current SAT, this means taking your highest Math score and your highest Evidence-Based Reading and Writing (EBRW) score across all test dates. Maybe you scored a stellar 750 on Math on your first SAT, but your Reading and Writing fell into the low 600s. To improve your EBRW score, you spent a few months prepping and managed to bring your EBRW score up 100 points! To your dismay, though, you focused so much on reading comprehension and grammar that you forgot some of your math skills, and your great math score dropped the second time around. If your colleges superscore, then you'd have nothing to worry about! They'd take your original 750 in Math and combine it with your new and improved EBRW score. Even though it took you a couple test sittings to achieve, you'd end up with a strong SAT score in all sections of the test for your college applications. While the superscoring policy is ideal for you as an applicant, it's not entirely an act borne out of generosity. Superscoring can benefit colleges by improving their ranking. By showing that their admitted students average higher SAT scores, their ranking may get higher as well. Before getting into the full list of colleges that superscore, there's another important consideration for current high school students. With the switch to the new SAT in 2016, how were colleges' superscoring policies affected? School Official Statement Admissions Site Boston College If you have taken the SAT more than once, Boston College will take the highest individual section scores from each date and combine those scores. BC Admissions Boston University Boston University will allow students to choose [the College Board's Score Choice]; however, we strongly recommend students submit their scores each time they take the SAT. BU has always considered a student’s highest SAT section scores in making admissions decisions and will continue to do so. BU Admissions Columbia If you take an exam more than once, you will be evaluated on the highest score you received in any individual section. Columbia Admissions Duke Students who have taken multiple tests may choose which scores to send to Duke. For students who elect to send multiple test scores Duke will use whichever score is highest. It only means that your child is in his transitional age and he is in need of your understanding and support. But how to handle such behaviour? Parents should become his close friends. First of all they should learn to respect his interests. Try to speak with him as often as possible, offer some parent-child activities like shopping or going in for sports. It is worth involving the child in discussing some family questions, just to show that he is a full member of the family. Moreover, children in their teens are very vulnerable when they are criticized in public, so try to avoid it. Parents should always be honest and sincere with their child; otherwise it would be unfair to require the same from him. Too authoritarian parents cant do any good to their children as well as parents who overindulge all the childs caprices. Overindulgence may lead to the same results as negligence. Anti-tuberculosis Drug-induced Liver Injury (ATLI) Effects Abstract Tuberculosis (TB) is world’s deadliest communicable disease, with 9 million incident cases and 1.5 million deaths globally in 2013. Most of the cases of TB were reported from Asian (56%), and African (29%) continents. In Saudi Arabia the annual incidence rate of TB ranged between 14 and 17/100,000. Two provinces, Makkah and Jazan showed the highest incidence around 20/100,000 over the last one decade. Jazan region showed more than double the incidence rate of TB compared to rest of the Southern province. Jazan share border with Yemen, and receives many illegal immigrants. Studies showed that non-Saudi Arabians had 2-3 times higher incidence of TB than Saudi national. The large number of Non-Saudis came from countries with high incidence of TB such as Bangladesh, Pakistan, India, Indonesia, Philippines, Yemen, Ethiopia, Somalia, Chad, Nigeria and other African countries. The active TB patients can be treated safely and effectively through the directly-observed therapy strategy (DOTS). DOTS is a short course of standard anti-TB treatment which consist of taking drug combinations of Isoniazid, Rifampicin, Pyrazinamide, Ethambutol and Streptomycin for 6–9 months. The drug isoniazid, rifampicin and pyrazinamide have the potential to induce liver damage.This anti-tuberculosis drug-induced liver injury (ATLI) ranges from mild to severe forms, and can even be fatal. The incidence of ATLI during standard anti-TB treatment range from 2.0% to 28.0% according to different populations. Moreover, ATLI reduce the efficacy of anti-TB treatment, as they may cause treatment failure, relapse and drug-resistance which could significantly reduce the effects of TB control. To the best of our knowledge, there is no known published data on the incidence of anti-tuberculosis drug-induced liver injury (ATLI) and risk factors from Saudi population. Identification of patients at increased risk for ATLI is important because hepatotoxicity causes significant morbidity and mort ality and may require modification of the therapeutic regimen. The aim of this study will to estimate the incidence of ATLI and the risk factors associated with anti-TB treatment. The findings of this study will allow us to enhance TB treatment, monitoring and control of the TB in KSA. Jazan region showed more than double the incidence rate of TB compared to rest of the Southern province. Jazan share border with Yemen, and receives many illegal immigrants. Studies showed that non-Saudi Arabians had 2-3 times higher incidence of TB than Saudi national. [3]. The large number of Non-Saudis came from countries with high incidence of TB such as Bangladesh, Pakistan, India, Indonesia, Philippines, Yemen, Ethiopia, Somalia, Chad, Nigeria and other African countries. Patients with active TB disease can be treated safely and effectively through the directly-observed therapy strategy (DOTS) which started in 2000. DOTS is a short course of standard anti-TB treatment which consist of taking drug combinations of Isoniazid, Rifampicin, Pyrazinamide, Ethambutol and Streptomycin for 6–9 months [4]. These drugs effectively kills the bacteria but it induced hepatotoxicity known as anti-tuberculosis drug-induced liver injury (ATLI) [5.tostmann 2008]. The ATLI ranges from mild to severe forms, and can even be fatal. The incidence of ATLI during standard anti-TB treatment range from 2.0% to 28.0% according to different populations. [5,6]. The incidence is higher in the developing countries (8% to 39%), compared to developed countries (3%–4%) (7-11). Moreover, ATLI reduce the efficacy of anti-TB treatment, as they may cause treatment failure, relapse and drug-resistance which could significantly reduce the effects of TB control. [5,6]. Many risk factors have been implicated for ATLI. These include older age, female gender, poor nutritional status, pre-existing liver disease, high alcohol intake, hepatitis B, malnutrition, hypoalbuminaemia and advanced TB (12-16). Inappropriate use of drugs, acetylator status, and recently, immunogenetic factor, have also been implicated (17,18). Infections with hepatitis C virus and human immunodeficiency virus (HIV) have also been said to increase the risk (19). It is very important to understand the risk factors of ATLI, in order to detect the adverse events earlier and deliver timely intervention. The identification of high-risk patients would be useful to allow early detection of hepatotoxicity and reduce the morbidity and mortality of this condition. Hence we plan this study to identify the risk factors associated with anti-tuberculosis drug induced liver injury in patients who receive anti-TB treatment. To the best of our knowledge, there is no known published data on the incidence of anti-tuberculosis drug-induced liver injury (ATLI) and risk factors from Saudi population. The aim of this study will to estimate the incidence of ATLI and the risk factors associated with anti-TB treatment. The findings of this study will allow us to enhance TB treatment, monitoring and control of the TB in KSA. 2. PROJECT OBJECTIVES This study aimed to estimate the incidence and risk factors of anti-tuberculosis Drug Induced Liver Injury (ATLI) in patient receiving anti-TB treatment in Jazan population. 1. To estimate the incidence of abnormal liver function tests (LFTs) in patient receiving anti-TB treatment. 2. To identify the risk factors associated with anti-tuberculosis Drug Induced Liver Injury (ATLI) in patient receiving anti-TB treatment. Review of Literature Incidence The first line drugs used to treat TB were isoniazid (INH), rifampicin (RIF), pyrazinamide (PZY) and ethambutol (EMB). Most of the TB patients tolerate the drugs but some develop hepatotoxicity known as anti-tuberculosis drug-induced liver injury (ATLI). [Forget 2006].The ATLI ranges from mild to severe forms, and can even be fatal. Data from the literature shows that the incidence of ATLI is 3.0% in Canada (Asia population accounted for 42%)[Yee 2003], China 2.5% [Shang 2011], 5.0% in Hong Kong[ Chang 2008], 5.3% in Singapore[teleman 2002], 16.1% in Taiwan [Huang 2003], 9.7% in Malaysia [Marzuki 2008],36% in Japan [Ellard 1978], 8-36 per cent in India [Parthasarthy 1986, Mehta 1990], 13% Iran [Baghaei]. The risk of developing ATLI was fivefold for hepatitis C patients, fourfold for HIV positive patients, and 14 fold for patients co-infected with hepatitis C and HIV [Ungo 1998]. Definition of ATLI The criteria for the diagnosis of ATLI in the absence of symptoms is elevation of transaminases up to 5 times the upper limit of normal (ULN) and in the presence of symptoms up to three times the ULN or twice the ULN of bilirubin in the blood [Saukkonen 2006]. Mechanism of toxicity Among Isoniazid, rifampicin, pyrazinamide and ethambutol, the first three drugs have the potential for hepatotoxicity with pyrazinamide (PZA) being the most hepatotoxic followed by isoniazid (INH) and rifampicin.[Yee]Rifampicin combined with PZA is more hepatotoxic than with INH.[ Jasmer 2002]Pyrazinamide contributes significantly to acute liver failure [ Durand 1995].The most important adverse effects of isoniazid are hepatic toxicity and potentially fatal drug-induced hepatitis[Nolan 1999], especially when associated with rifampicin. The frequency of occurrence of isoniazid-associated hepatitis depends on age. Other factors linked to a predisposition to isoniazid-associated hepatotoxicity include alcohol abuse, use of illegal drugs and a previous history of liver disease. Clinical features ATLI usually take place in the first 2 months of treatment but it may happen at any time during the treatment period. Clinical and biochemical features of ATLI are difficult to differentiate form viral hepatitis [Mitchell 1976]. The signs and symptoms of ATLI are jaundice, abdominal pain, nausea, vomiting and Weakness. Risk factors for ATLI Many risk factors have been implicated for ATLI.
